| A. You may start using the benefits once the card has been activated. For activation of the card, please present this card at the help-desk/reception of your nearest Max Facility and fill-in your family's details in the enrolment form. |

| Q. Will the card be applicable to families with 4 or more members? |

| A. Max Healthy Neighbourhood card is valid upto 6 members of a family (names to be mentioned in the enrollment form). Should you have senior citizens in your family, the benefits can be extended to them as well. |

| Q. Does every member of the family receive an exclusive Max Healthy Neighbourhood card? |

| A. One card will be issued per family. Details of family members and age are to be filled in by the card holder, in the enrollment form. |

| Q. What is the validity of the Max Healthy Neighbourhood card? |

| A. This card is valid till 31st December, 2010 but is subject to renewal. |

| Q. Can I gift this card or pass it on to my friends and relatives? |

| A. Max Healthy Neighbourhood card is non-transferable. |

| Q. In case I shift residence, will the benefits hold good? |
|
| A. The benefits hold good irrespective of your change in residence. |

| Q. My mother is 61 years old. Is she entitled to the Senior Citizen mentioned benefits in the Max Healthy Neighbourhood card? |

| A. Senior Citizen benefits will be applicable to those above 65 years. |
