Bleeding, pain and itching in the anal area are a few uncomfortable situations, which will definitely make you worry. You should not wait rather rush to the doctor to avoid any complications. To know what is causing this: Let us read below:

Piles

Enlarged veins that are located at the junction of rectum and anus are known as Hemorrhoids, Mulvyadh or Bavasir.

What are the Signs and Symptoms of Piles?

You may witness:

  • Bleeding during a bowel movement. There may be fresh blood at the end of passing stools or you may witness blood on the tissue.
  • You may feel a lump outside the anus.
  • Piles may cause itching in the region aka Perianal pruritus.
  • There may be a sensation of stool being ‘stuck’ in the anal region after a bowel movement.
  • Rarely, there may be a dull pain.

As a complication, the Piles sometimes develop a clot (Thrombosed Hemorrhoid). This may cause the development of sudden pain which worsens on sitting or during movement.

Few Causes of Piles:

It is challenging to examine the exact cause in every individual. However, the most likely causes are as follows:

  • Long-standing constipation
  • Excessive straining during bowel movements over a long period of time, or very hard stools that are not excreted smoothly
  • Family history of piles
  • Faulty toilet habits – for example, some people have a habit of spending too much time in the toilet, straining while they read, work on cell phones etc.
  • Pregnancy and Childbirth – An increase in the hormone Progesterone causes the blood vessels to relax which leads to pooling of blood in them. Also, the pressure of the growing uterus on the vessels prevents them from emptying completely
  • An unhealthy lifestyle – Lack of exercise, stress, alcohol, smoking
  • Obesity – Piles is more common in people who are overweight
  • Lifting heavy weights
  • Ageing – the tissues in the lining of the anus may become less supportive with age

Fissures

An anal fissure is a small tear or a crack in the lining of the anus.

What Causes Fissure?

The most observed causes of fissures are passing large or hard stools, straining during childbirth, or experiencing bouts of diarrhoea. Besides the ones mentioned, the other causes are:

  • Decreased blood flow to the anorectal area
  • Anal Cancer
  • Overly tight or spastic anal sphincter muscles
  • HIV
  • Crohn’s disease or inflammatory bowel disease
  • Tuberculosis
  • Herpes
  • Syphilis

Know the Signs and Symptoms of Fissure

An anal fissure may cause one or more of the following symptoms:

  • Visible tear in the skin around the anus
  • Skin tag, or small lump of skin, next to the tear
  • Sharp pain in the anal area during bowel movements
  • Streaks of blood in stools or on tissue paper
  • Burning or itching in the anal area

Fistula

An anal fistula is a small channel that can develop between the end of the bowel and the skin near the anus.

What causes Fistula?

Most fistulas result from an anal abscess. A small number of fistulas may less frequently be caused by other processes such as Crohn’s disease, sexually transmitted diseases, trauma, tuberculosis, cancer or diverticulitis.

Signs and Symptoms of Anal Fistulas

  • Pain, which is usually constant, throbbing and worse when sitting down
  • Constipation or pain associated with bowel movements
  • Skin irritation around the anus, including swelling, redness and tenderness
  • Discharge of pus or blood
  • Fever
  • Pilonidal Cyst

Pilonidal cysts are the sacs filled with hair and skin debris that are formed at the top of the crease of buttocks above the sacrum. A painful abscess can form if the cyst and overlying skin become infected.

What are the Causes of Pilonidal Cyst?

Dr. Manish Aggrawal, Senior Consultant, Department of General Surgery and Robotics, says that cysts arise due to trauma caused to that area, resulting in ingrown hair. Pilonidal cysts often contain hair when excised, but hair follicles are not seen in them.

Know the Signs & Symptoms of Pilonidal Cyst

  • Pain, redness and swelling at the bottom of the spine
  • Bad smell from the pus
  • Pus or blood draining out of it
  • Tenderness to the touch

What should be the treatment for these conditions?

  • Eat plenty of fibre such as fruit, vegetables, cereals, whole grain bread, etc,
  • Drink plenty of water
  • Fiber supplements like ispaghula, methylcellulose, bran or sterculia
  • Avoid painkillers that contain codeine such as co-codamol, as they are a common cause of constipation. However, simple painkillers such as paracetamol may help
  • Good toileting habits
  • Ointments, creams and suppositories – Various preparations and brands are commonly used. They do not cure piles. However, they may ease symptoms such as discomfort and itch
