Artificial Urinary Sphincter: Treatment for Complete Urinary Leakage

Dr. Anupam Bhargava, Chairman, Urology, Max Smart Super Speciality Hospital says he felt sorry for a fifty-eight years old man who underwent surgery for prostate and was facing the problem of leaking urine all the time. Though this is a very rare complication of prostate surg ut it can happen. He had to use three to four urinary diapers a day. He was depressed and was totally frustrated with life.

I offered him some remedial measures with medicine and few pelvic muscle exercises. He declined to take medicines as he has been using them since the last one year but all was in vain. The second option was to inject some bulking agent in the urinary passage at the site of valve. The problem with this procedure was it’s unpredictability.

The option of having urinary tube all the time for a young active man was not a good option. I told him to use an external device. This is like a pinchcock, which can be used on phallus. Patient was not satisfied with this mode of treatment also.

At last, I told him that I will be fixing an artificial valve. The surgery will be an open surgery and will last for an hour or so. I told him that he will have to learn how to press the button placed in his scrotum to control the valve to which he happily agreed. The surgery was performed and he was sent home for two weeks, still leaking. He was asked to come back after two weeks to activate the valve. He came back and the valve was activated. To his delight, he felt there was full control on urination. He came back to my OPD after three months happy and satisfied.

This procedure is called fixing Artificial Urinary Sphincter. This surgery is indicated for those people with a total urinary leakage or those who have had surgery for cancer prostate or after benign prostate surgery. The only drawback is it is expensive.
