A middle-aged man suffering from Ca Splenic Flexure of the colon with liver metastasis, visited Max Super Speciality Hospital, Dehradun, with acute large bowel obstruction.
In the first stage, a Left Hemicolectomy (Hartmann procedure), with an end Transverse colostomy was done. The patient then underwent seven cycles of Chemo. Repeat PET CT showed a reduction in the size of Liver Mets, which were limited to the right lobe of the liver, with no metabolically active lesion elsewhere.
In the second stage, End Colostomy was reversed and a right lobe hepatectomy was performed in the same sitting.
A Right Lobe Hepatectomy is, anyway, a very challenging surgery. In this patient, it was even more difficult due to the variation in his liver anatomy. The patient was discharged in stable condition & has recovered well.
