Two complex cases treated with TAVI/TAVR

Case No 1

Mrs. M.R, 84yr elderly lady presented with breathlessness on minimal exertion for which she had multiple admissions in last few months. She had been suffering from high blood pressure and was also on thyroid with sugar medications. She had suffered a stroke in the past and has also undergone mitral valve replacement surgery with permanent pacemaker implantation in the past. Her 2D echo showed severe aortic stenosis, with calcified valve.

The conventional treatment for such patient is aortic valve surgery. But due to age, co morbid conditions and previous heart valve surgery, she was considered very risk for surgery and surgical option was ruled out. Therefore she was planned for TAVR (Trancatheter aortic valve replacement surgery), where through femoral route, new aortic valve is implanted over pre-existing valve. This does not require any general anesthesia and the whole procedure takes only couple of hours. Patient underwent successful TAVR with Evolute Pro 29mm (Medtronic) valve. Post procedure recovery was uneventful.

Case No 2

Mr. C.L.S., 75 yr elderly gentleman presented with breathlessness on exertion. He was already a known case of coronary artery disease and had undergone CABG surgery in 2007 and then suffered inferior wall myocardial infarction and underwent stenting in 2016. His 2D echo showed severe aortic stenosis, with calcified valve.

The conventional treatment for such patient is aortic valve replacement surgery. But due to age, and previous CABG surgery, surgery was considered high risk. Therefore he was planned for TAVR (Trancatheter aortic valve replacement surgery), where through femoral route, new aortic valve is implanted over pre-existing valve. This does not require any general anesthesia and the whole procedure takes only couple of hours. Patient underwent successful TAVR with Myvalv 24.5mm (Meril life sciences) valve in April 2022. Post procedure recovery was uneventful.

TAVR is procedure of choice in high risk aortic stenosis patient, and now its usage has been extended to low risk patients also as it gives comparable result to surgical valve replacement with shorter hospital stay and lower morbidity & mortality.
