Sentinel Lymph Node Biopsy (SLNB)

While performing a sentinel lymph node biopsy, the aim is to identify the ‘sentinel’ or ‘guard’ node, the first node to which the breast cancer is likely to spread. This procedure is facilitated by injecting a radioisotope and a blue dye and identification of the nodes in the theatre by looking for ‘hot’ (picked up by a gamma probe and counter) and ‘blue’ nodes as also also enlarged nodes. These nodes varying in number from 1-4 lymph nodes are sent to the pathologist to assess if there is any evidence of involvement of the node by disease. If any of them are involved, then the usual practice is to remove all the lymph nodes in the armpit. The benefit of doing sentinel lymph node biopsy is to reduce shoulder dysfunction and reduce the incidence of arm swelling to less than 5% which is almost 20-25% with removing all the lymph nodes.
