When Do You Need A Kidney Transplant?

The human body has a pair of kidneys on each side of the spine. The kidney keeps the blood clean by filtering waste from it. The waste is discarded in the form of urine. If the kidney fails to perform its task efficiently, waste will accumulate in the body which is life-threatening. The accumulation will lead to a rise in blood pressure and the emergence of end-stage kidney disease. End-stage kidney disease may occur due to several factors. However, the most common ones are:

  • High and uncontrolled blood pressure
  • Polycystic kidney disease
  • Diabetes
  • Chronic glomerulonephritis
  • Lupus
  • Repeated urinary infections

Kidney failure is a severe condition and requires immediate medical attention. Two ways to treat kidney failure are - dialysis and kidney transplant. The need for a kidney transplant is when the diseased organ is replaced with a donor’s healthy kidney. A kidney transplant is suitable for everyone, irrespective of age. But the doctor will conduct various tests, check medical history, undertake dental evaluation and follow other processes to determine whether the patient is suitable for a kidney transplant. The doctor will recommend dialysis for patients affected by end-stage kidney failure. In dialysis, a machine removes waste from the patient’s bloodstream. Patients who opt for dialysis have to get the treatment for their entire life. The average life expectancy with dialysis is between five to ten years.

Kidney Transplant Surgery and Procedure

You can receive a healthy kidney from a deceased or a living donor. If the donor is deceased, you will be put on the waiting list by the hospital where the transplant is to be performed. When a donor is identified, you will be notified and will have to reach the hospital within a specified amount of time. At the hospital, the medical team will first conduct an antibody test. For this, they will take a blood sample. This will ascertain whether the patient is suitable for transplant. The Surgery involves a number of small incisions (1–3 cm each), through which instruments are easily inserted to remove the kidney. A larger incision is then made to take out the kidney. Once the surgeon gives a go-ahead for the transplant, you will be administered general anesthesia so that you do not experience any pain. When you have reached a state of deep sleep, the surgeon will make a cut in your abdomen. The donor kidney will be planted inside the patient’s body through this cut. It will then be connected to the blood vessels and bladder. The patient’s original kidneys will not be removed. But if the patient experiences problems like high blood pressure, etc., the doctor might consider removing the original kidneys. The kidney transplant Surgery process in India is the same as in Western and US hospitals.

Post Kidney Transplant Care

A kidney transplant will enable you to lead a long and healthy life. But to ensure that the new organ works well, you need to care for it and take certain precautions.

  • Monitor your condition regularly

Post-surgery, the hospital will monitor your condition. If the staff feels your condition has stabilized, they will shift you to a regular room. In some cases, the kidney starts working immediately. But other patients may have to wait for a few weeks or a month or two for the kidney to resume its duties.

  • Keep taking prescribed medicines

A kidney transplant means that a new organ is to be planted in your body. So, there is a likelihood that your body may reject the new kidney. To prevent this complication from arising, the surgeon will recommend immunosuppressant drugs. It is essential to take these drugs daily.

  • Follow the instructions given by the doctor

Once the surgeon finds that you have recovered well, you will be discharged from the hospital. But your doctor will give you specific instructions which you will need to adhere to.

  • Take proper care at home

Proper care should be taken at home so that no harm comes to the kidney. Eat a healthy diet and keep your surroundings clean and hygienic. It is necessary to avoid contracting infections as these have the potential to harm the new kidney.

  • Follow up with doctor regularly

For the first year, regularly follow up with your doctor. The doctor will continuously monitor the kidney, and if any complication is noticed, steps will be taken to address the issue.

Kidney Transplant Cost in India

The cost of kidney transplant operation in India is determined by the patient's medical condition, post-operative care, and the location of the surgery. These fees are generally clear, and patients can discuss them with their caregivers and hospital staff prior to operation. Furthermore, kidney operation cost in India is less than in most Western countries. As a result, patients travel to India in search of expert surgeons, cutting-edge medical facilities, and world-class care at a low cost. Factors contributing to the kidney transplant treatment cost in India include:

  • The reputation of the hospital, quality of the medical infrastructure, technologies used, and expertise of doctors.
  • Complication that may be experienced after the surgery
  • Medication to be taken after surgery
  • Dialysis cost
  • Patient’s age
  • Type of admission room
  • Recovery and rehabilitation
  • Lab and examination tests

Is Kidney Transplant Legal in India?

According to the Transportation of Human Organs Act, passed in 1994, organ donation is legal in India. The donation can be an in-live-related donation or an in-live unrelated donation. In living related, the organ is donated by a living person, which can be any family member. In living unrelated donation, the organ is donated by a person not related to the patient. Nevertheless, they are emotionally related to the patient like friends, etc. The patient needs to register with a hospital specializing in kidney transplants for deceased organ donation. The patent will be on a waiting list, and when their turn comes, the operation will be conducted.

Rheumatology: It is a sub-speciality of medicine that deals with the diagnosis and treatment of diseases related to rheumatic or musculoskeletal disorders.

Rheumatic diseases: These diseases are characterised by inflammation that affects the connecting or supporting structures of the body — most commonly the joints, but also sometimes the tendons, ligaments, bones, and muscles. It encompasses the following spectrum of diseases:

  • Arthritis: Joint Inflammation
  • Connective Tissue Diseases:  Inherent or autoimmune diseases of connective tissues that causes inflammation
  • Musculoskeletal disorders: Diseases of bone, joints, cartilage, muscles, ligaments and tendons

The major indicators of Rheumatic diseases are:

  • Joint pain and  swelling
  • Tightening of the skin
  • Redness in the area of joint
  • Loss of motion in a joint or joints
  • Inflammation in joints
  • Swelling in the affected area
  •  Warmth in a joint or affected area
  • Fingertips turning blue
  • Dryness in eyes, mouth and other body parts
  • Fever
  • Fatigue
  • Loss of weight
  • Rashes on different body parts

The symptoms may vary from person to person. A doctor can examine a person, evaluate the symptoms and recommend proper treatment plan accordingly.

Some of the major conditions treated are:

  • Osteoarthritis
  • Rheumatoid Arthritis (RA)
  • Reactive Arthritis
  • Juvenile Idiopathic Arthritis
  • Spondyloarthropathies
    1. Ankylosing Spondylitis (AS)
    2. Psoriatic Arthritis (PsA)
  • Lupus
  • Gout
  • Scleroderma
  • Sjogren’s Syndrome
  • Polymyalgia Rheumatica

Rheumatic diseases are generally chronic conditions that are not fully curable. However, there is effective management plan available for most of the conditions associated with such diseases.

  • The doctor will evaluate the patient’s condition on the basis of:
    1. Intensity of pain
    2. Location of pain, including symptoms like stiffness and swelling
    3. Condition throughout the day
    4. Body positions, medication and other things that provide relief from symptoms
    5. Factors that worsen the pain
  • The  doctor will also take into consideration any family history related to autoimmune diseases
  • The patient may have to undertake:
    1. Urine test
    2. Blood tests
    3. Imaging tests

These tests will help detect markers of inflammation, abnormal blood counts, antibodies associated with certain diseases, and abnormal organ function, cartilage loss, fluid build-up, soft tissue tears, bone or joint erosion and damage, among other things.

  • According to the evaluation, the patient might be recommended the following treatment plan:
    1. Medication:
      1. Non-narcotic analgesics
      2. Narcotic analgesics
      3. Non-steroidal anti-inflammatory agents (NSAIDs)
      4. Corticosteroids
      5. Disease-modifying anti-rheumatic drugs (DMARDs)
      6. Biologic response modifiers (biologics)
    2. Specific exercise regimes to increase muscle strength and joint flexibility
    3. Physical therapy
    4. Hot and Cold therapy
    5. Relaxation therapy
    6. Occupational therapy
    7. Support weak joints using braces, splints, and assistive devices
    8. Surgery (particularly for arthritis)

Risk factors that may be avoided:

  • Obesity: Being overweight can damage the  knee joints, thus making a person prone to rheumatic diseases
  • Injury: Damage to joints due to injuries or accidents are more likely to convert into arthritis
  • Infection: Infection in joints can result in reactive arthritis
  • Occupation: Job that involves repeated bending or squatting can elevate risk of knee arthritis

One can easily reduce the risk of Arthritis by taking proper care of health, maintaining an active & healthy lifestyle and avoiding delay in getting routine health check done.
