Stroke can strike at any age: early treatment can save life

A 32-year-old female came to the emergency with sudden onset right haemiparesis with global aphasia of 45 minutes. After initial triaging in emergency, a stroke code was alerted.

A green corridor was provided for the patient to be shifted to the MRI room in less than 5 minutes. MRI of the brain revealed acute infarct in left basal ganglia which was flair negative. Since patient had come within the window period, IV thrombolysis with inj Actilyse 0.9mg/kg body weight was done after taking informed consent.

The door to needle time was 19 minutes only. CT Angiography was also done which showed left middle cerebral artery M2 occlusion. Since large vessel occlusion responds poorly to IV thrombolysis, the endovascular team was alerted. But patient started significantly improving after the infusion (IV thrombolysis) was completed, hence mechanical thrombectomy was not considered.

After 24 hours CT of the head did not reveal any haemorrhagic complication and therefore, antiplatelets were started. Patient made extremely good recovery and started walking independently, with complete recovery of speech function at the time of discharge. Worldwide, there has been a 40% increase in the incidence of stroke among young adults, reported over the past decades. In India, stroke in young adults constitutes around 15-20% of all stroke cases.

Since this is the most productive period of life, any delay in treatment of stroke can cause lifelong disability/death. Early treatment within the first 4.5 hours can save life and prevent disability. IV thrombolysis is the standard of care for ischemic stroke within 4.5 hours.
